Personal health record

Personal Health Record Mobile App and Website - LIVE and available to download

Personal Health Record is a mobile app and website which will allow you 24/7 digital access to a summary of your confidential health and social care record held in One Health and Care. It will enable wider access to your information and self-management of your own health and care as updated by health and social care professionals who are directly involved in your care.

This will enable you to:

  • View a summary of your medical and social care records
  • Record information to self-manage your health and wellbeing
  • Access advice and support for your health and wellbeing

There are a number of health and care apps which are available to support health and wellbeing, and it has been recognised that this could be confusing for the user. There will be a move to pull patient-facing apps where technically possible into the NHSApp to form a common patient gateway. The NHSApp and Personal Health Record will work alongside each other to give the user a blended experience, and be referred to as PHR.

All GP data available within the NHSApp will remain the same and will be complimented with data from health and care providers.

As part of the launch of Personal Health Record, a communication campaign will be undertaken prior to the app being available for download.

  • How is the Personal Health Record mobile app and website different to other solutions?

Personal Health Record will display information from the shared care record One Health and Care; giving you a summary view of your record which will include data from your GP, as well as any secondary care, community services (including mental health) and social care providers involved directly in your care.

    • University Hospitals North Midlands (UHNM) are developing the ability for you to see your appointment letters within the mobile app and website. You will be asked if you wish to receive your letters electronically by the e-appointment team at UHNM.  Viewing appointment letters via Personal Health Record will improve the time taken to inform you of your appointments, helping you to manage your time and receive your care more efficiently. UHNM are working with a pilot group of patients to explore the use of the app and website to drive future requirements, and to learn from their experience.
    • Another feature of the Personal Health Record is the ability to maintain your health diary. You will be able to self-record information such as blood pressure, blood sugar, weight, personal goals and over-the-counter medication. There is also the ability to link up information from fitness wearables such as Fitbit, Apple and Garmin. The Personal Health Record service is not a real-time messaging and alerting system, and health and social care professionals cannot view the data you add to it.
    • Please remember – information that you can add to the app will then be available for data analysis under secondary use purposes. At no time will the information viewed within the Personal Health Record service be passed to organisations for marketing or sales purposes, or for any commercial use.
    • You may have access to websites and apps linked to your GP practice, and some of the features include the ability to request repeat prescriptions and appointments. Personal Health Record does not include such features and therefore you will need to continue using your GP-facing apps until such a time when the PHR is linked to the NHSApp.
  • How can the Personal Health Record app be downloaded?

The Personal Health Record app will be free to download from the App Store (Apple devices) and Google Play (Android devices) in 2021. You will also be able to access it through your internet browser on your PC, tablet, or laptop when the new website is live.

  • How is access managed to the Personal Health Record?

You will be able to gain access to Personal Health Record with a secure NHS login. If you have an account, you will be prompted to login using your existing details.

    • To set up your NHS Login you will need:
    • an email address
    • a UK mobile phone number

NHS login services will send you an email containing a link. Select the link to confirm your email address. Then return to the website or app you want to access and enter your NHS login email address and password. Here, you will be asked to provide your mobile phone number.

A 6-digit security code will be sent by text message to your mobile phone. You will be asked to enter this code to continue. A security code will be needed to log in every time, unless you select ‘remember this device and stop sending security codes’.

Right now, you cannot set up NHS login without a mobile phone. NHS Digital are working on this and hope to offer a solution to users who do not own a mobile phone in the future.
