Claire Wagner - Personalised Care Roles Facilitator

Claire has a professional background in teaching, training and youth work. 

As an experienced former case worker within substance misuse, Claire worked for many years in primary care settings, co-ordinating and managing the delivery of substance misuse treatment in general practice and providing training for the RCGP. She went on to become a Training and Development manager specialising in substance misuse and led training and clinical supervision groups in Motivational Interviewing and Person-Centred approach for staff. As a Senior Practitioner with lead responsibility for children and families, Claire trained on behalf of the Telford and Wrekin Safeguarding Partnership and has since worked freelance as a consultant trainer for a range of charities across the UK. Claire currently teaches public health at Shrewsbury College, part-time at Level 3 and on foundation degree. 

Prior to undertaking her new role as Personalised Care Roles Facilitator, Claire was a Healthy Lives Advisor for both adults and Children and Young People. She is a great believer in the value and benefit that personalised care roles can bring to individuals, communities and health and social care systems. Claire will utilise her experience of key work, workforce development and multi-disciplinary working in primary care to support the progression of these roles in general practice, schools and communities.  

In her spare time, Claire enjoys running, cycling and generally being outdoors and spending time with her two daughters.
